Family assessment: Caseworker will meet with family and assist family with intake, and assessment. This assessment is to be completed upon families entering the program. This assessment is an important component, and is used to assist family in successfully reaching it's goals of self-sufficiency.

Mediation: Caseworkers will provide mediation and appropriate crisis intervention, upon request of client. Mediation is only to be used where both parties are in agreement to this process. Mediation is not to be used in domestic violence situations or when there is a history of violence between the two parties.

Collaborative Goal setting/evaluating barriers: Caseworker and family will collaboratively create long term and short term goals. Caseworker will discuss and evaluate any barriers to reaching Families determined goals.

Identification of strengths: Caseworker and Family will work together in evaluating and identifying the strengths of the family unit. With the identification and implementation of strategies, Caseworker and family will create a realistic approach to obtaining goals.

Family advocacy: Caseworker will use his/her expertise to provide professional advisement into appropriate referrals to county or out of county agencies. Caseworker will intervene when deemed appropriate to act as a liaison with consumer's service providers.

12-month follow-up/tracking: Caseworker will contract with family to work with families on on-going need's and goals. This program will last up to 12 months with the possibility of extension, if Caseworker and family feel there are further needs and goals to be met. Caseworker and family will be required to meet at home or a designated place twice a month to work on goals and needs.

Provide a Scaffold/Supportive Approach to Empowerment: Through the identification of strength's caseworker will assist family in utilizing these strength's as to create a structured realistic approach to growth. Caseworker will provide advisement, resources, encouragement, and assistance, where there is a strong need assessed. Caseworker will maintain a professional relationship with family.
